POEM stands for Patient-Oriented Eczema Measure:

A tool recommended by the NHS which is recognised by GPs and dermatologists
POEM scores help you to offer advice and take the appropriate action to support your patients to manage their eczema
By answering a few simple questions POEM monitors eczema severity based on how it has personally affected your patients over the past week
The app calculates a POEM score ranging from 0 to 28, that corresponds to five categories of eczema severity (clear to very severe)
So your patients can share how severely their eczema is ‘bothering’ them in a clearly defined way that you understand and can act on

Based on the NHS and internationally recognised EASI (Eczema Area and Severity Index) scoring system:

Helps your patients and you to identify what is triggering their flare-ups so they can avoid them, and learn how to manage them
Gives a score by body area based on intensity of skin redness, thickness, the level of scratch marks, and how leathery or lined (lichenified) the region is
Your patients can even add in photos of the affected area and any relevant notes, which can be shared with you
The app automatically records UV and air quality at the time in your patients area, as these can be factors that are potentially affecting their eczema
You now have the full picture of the severity of your patients eczema symptoms, so that you can let them know what they need to do to get their condition back under control

Based on what your patient tells the app, about the treatments that they use as recommended by you:

Your patients personalised written action plan is their ‘road map’ to improving and managing their eczema
By following their plan each day they can achieve healthier skin with less painful flare-ups
Their personal written action plan is divided into 3 stages, depending on eczema severity, with each one guiding them to the best way to treat their eczema, including when to seek help
Your patients can better understand and manage their condition by viewing the in-app videos and information sheets: e.g. about eczema, how to use emollients, as well as tips and advice
Notifications and supportive advice and reminders are sent directly to your patients phones when required
